Contract Specifications – Terms and conditions for service providers working with Kent County Council.

The Kent Good Health Group has worked with Kent Hospitals to agree these Hospital Access Sheets.  They are designed for people living in the community, or services, to have points of contact when going into hospital for either planned visits or in an emergency. Using these contacts, they can let hospital services or the Community LD Teams know if they need additional help or support.
